2017-2018 School Health Requirements

Dear Parent/ Guardian:

 

Illinois State Law requires that every child entering Pre-School, Kindergarten, Second, Sixth, Ninth, and Twelfth grades to have the following completed documents on file with the school.  These forms can be turned in at school registration or at the start of the school year.  These are due no later than October 15, 2017 otherwise your student will be counted as non-compliant on the report given to the state.  

 

Preschool:

  • Physical examination (completed form including: BMI & Diabetes Screening)
  • Lead Screen
  • Current Immunization record

 

Kindergarten:

  • Physical examination (completed form including: BMI & Diabetes Screening)
  • Lead Screen
  • Current Immunization record (including:  4 Dtap, 4 Polio, 2 MMR, 2 Varicella Vaccinations)
  • Dental Examination
  • Vision Examination

 

Second Grade:

  • Dental Examination

 

Sixth Grade:

  • Physical examination (completed form including: BMI & Diabetes Screening)
  • Current Immunization record (including 1 dose of the Meningococcal vaccine, 2 doses of Varicella, 1 dose of Tdap)
  • Dental Examination

 

Ninth Grade:

  • Physical examination (completed form including: BMI & Diabetes Screening)
  • Current Immunization record

 

Twelfth:

  • Current Immunization record (including 2 doses of Meningococcal vaccine.  Only 1 dose is required if the first dose was received at 16 years of age or older.)

 

 

It is advisable to get your appointments for physicals early as the doctor offices get busy in August.  Please return your completed forms to the appropriate school office by the start of school or during school registration. Thank you for your cooperation.
